I declare that the above information are correct and true to the best of my knowledge. I am prepared to accept the possibility of injury, and will not hold Wiltshire Brazilian Jiu Jitsu Academy or any of its coaches/instructors liable for any injury incurred or loss or damage to property. I will hereby abide to the rules and code of conduct set by Wiltshire Brazilian Jiu Jitsu Academy. I am mentally and physically fit enough to undertake Martial Arts training. I agree to a licence fee per member paid yearly which will be required to be active to attend class. I understand that Wiltshire Brazilian Jiu Jitsu Academy reserves the right to decline my membership application or suspend/cancel my membership at any time.
